Extrakare accepts most major health insurance plans. Insurance coverage usually requires a dispensing prescription for the item(s), evidence of a recent face to face evaluation and a detailed written order (or CMN). Certain items require specific diagnostic test be performed to ensure coverage. For example, most insurance companies require pulse oximetry or an arterial blood gas for oxygen. Most insurance companies require a facility-based or home sleep study for PAP devices and supplies.
Dispensing prescriptions can be faxed to Extrakare at (770) 449-3336. An Extrakare representative will contact your office if additional information is required to ensure the patient is eligible for insurance coverage.
